Step 1 - Becoming a New Patient
Becoming a new patient is easy at Synaptic Chiropractic Center.
The first step is to choose a location in Ohio or Massachusetts.
Once a location is selected, please call the office, with your insurance card available, and an assistant will be excited to help you schedule an appointment.
Prior to the first appointment, Synaptic Chiropractic Center will contact the insurance company and verify benefits on your behalf.
Step 2 - Forms and Paperwork
After step 1 is completed, the assistant will direct you to the ON-LINE FORMS section of the website.
Providing the forms on-line allows the patient the following conveniences:
- Patients are able to complete forms in the comfort of their own home prior to the first visit.
- Completing the forms on-line helps decrease the amount of time required at your first visit.
- Patients are able to print forms and fill them out by hand. The forms could then be brought to the first appointment or faxed ahead of time.
- Online submission via a secure encrypted network
If you do not have access to a computer, the same documents will be made available when you arrive on your first visit.
Step 3 - Previous Diagnostic Studies
If you've had any prior diagnostic studies done (i.e. x-rays, MRI, blood work), please bring them in if available.
If you do not have the images available, please let the assistant know so that they may provide you with a Records Release Form.
This form allows Synaptic Chiropractic Center to request the studies from your previous provider.
Simply bring the completed form on your first visit or fax it back if possible.
This information will help the doctor while investigating the cause of your presenting condition.
